My first thought is, “What is the health of his cardiovascular system?” The delicate blood vessels of the penis are often the first to show signs of the systemic vascular damage that leads to heart attacks and strokes. It can be the initial presentation of a much more serious, life-threatening condition. Therefore, a thorough investigation for cardiovascular risk factors is not just recommended; it is essential.

The Staggering Prevalence of ED

The statistics surrounding ED are truly astonishing. It is a condition that grows more common with age, and its prevalence is steadily rising. As of 2025, it was estimated that over 150 million men worldwide were affected by ED, and that number has undoubtedly grown. Age is a primary factor. Consider this general rule of thumb, which I call the “law of tens”:

  • At age 40, approximately 40% of men experience some degree of ED.
  • At age 50, that number rises to about 50%.
  • By age 70, as many as 70% of men are affected.

This progression is often linked to the natural decline in hormonal levels, particularly testosterone. While low testosterone does not directly cause ED in isolation, it is a significant contributing factor that lowers libido and makes achieving an erection more difficult. Men who present with low testosterone almost invariably report problems with erectile function.

The Four Pillars of ED Etiology

Erectile dysfunction is not a simple, single-cause issue. It is a multifactorial condition that we can break down into four main categories of causes.

1. Vasculogenic (Blood Flow) Causes

Another powerful tool in our regenerative arsenal is Platelet-Rich Plasma (PRP).   • ESWT Primes the Tissue: I think of shockwave therapy as “tilling the soil.” It breaks up old, fibrotic tissue, stimulates an initial wave of healing, and increases tissue permeability, making it more receptive to subsequent treatments.
  • PRP Plants the Seeds: Following up with a PRP injection is like “planting the seeds of regeneration” in the newly prepared soil. The concentrated growth factors from the PRP can then work their magic in an environment optimized for healing and growth.

This combined approach attacks ED from multiple angles: ESWT primarily targets neovascularization and blood flow, while PRP provides a broad-spectrum boost to tissue and nerve regeneration. For clinics equipped to perform injections, offering this combination therapy can provide patients with the most comprehensive and robust restorative treatment available. Even for non-injecting practitioners, such as many in the chiropractic field, ESWT alone is a powerful and highly effective modality.

Both therapies are minimally invasive and have an excellent safety profile. Patients may experience some minor, localized discomfort or bruising at the treatment site, but there are no serious systemic side effects associated with these treatments, unlike many pharmaceutical options. By focusing on healing the underlying pathology, we are not just treating a symptom; we are fundamentally treating the cause of the problem.

The Foundational Role of Hormones: A Deep Dive into Testosterone

As we navigate the journey of regenerative therapies for ED, there is a foundational layer of health that we must address first: hormonal balance. I cannot overstate this. If you are a practitioner considering offering peptide therapies or other advanced treatments, or a patient seeking them out, you must first check the hormones.

In my clinical practice, I have seen it time and again: a man comes in, frustrated that the peptides he has been trying are not working, or his ED is not improving despite his efforts. The first question I ask is, “Have you had your hormones checked?” More often than not, the answer is no. You must ensure that the foundational systems of the body—thyroid, adrenal (cortisol), and sex hormones (testosterone, estrogen)—are optimized. Otherwise, you are building your therapeutic house on a foundation of sand.

Let’s focus specifically on testosterone, as its decline is a central feature of male aging and a major contributor to the symptoms that bring men into my office.

The Alarming Prevalence of Low Testosterone

It is truly earth-shattering how many men are walking around with clinically low testosterone levels and do not know it. It is estimated that nearly 30 million men who are affected by ED also have low testosterone. As we discussed with the “law of tens,” the prevalence increases dramatically with age. Low testosterone, or hypogonadism, contributes to a constellation of debilitating symptoms that men often mistakenly attribute to just “getting older”:

  • Constant fatigue and lack of energy
  • Poor memory and “brain fog”
  • Decreased libido (sex drive)
  • Depressed mood or irritability
  • Erectile dysfunction or delayed ejaculation
  • Loss of muscle mass and strength
  • Increased body fat, especially around the waist
  • Diminished body and facial hair

Overcoming the Barriers to Diagnosis

One of the most significant challenges in diagnosing and treating low testosterone is the significant stigma that still surrounds men’s health. Guys being guys, many are reluctant to come into a doctor’s office and admit, “My stuff is not working,” or “I feel weak and tired.” There is often a deep-seated cultural or personal belief that needing help is a sign of weakness. They may also be concerned about relationship dynamics, not wanting their partner to know they are struggling with sexual problems.

To overcome these barriers, our clinic utilizes validated screening questionnaires. These tools allow men to privately and honestly answer a series of questions about their symptoms. If they check a few key boxes, it opens the door for a non-confrontational, professional conversation about their health. These questionnaires are an invaluable way to initiate a dialogue about sensitive topics that men might otherwise avoid.

The Physiology of Testosterone Production: The HPTA Axis

To understand what goes wrong in hypogonadism, we need to understand how testosterone is normally produced. This process is governed by a delicate feedback loop called the Hypothalamic-Pituitary-Testicular Axis (HPTA).

  1. Hypothalamus: The process starts in the brain. The hypothalamus releases Gonadotropin-Releasing Hormone (GnRH) in a pulsatile fashion.
  2. Pituitary Gland: GnRH travels a short distance to the pituitary gland, stimulating it to release two other key hormones: Luteinizing Hormone (LH) and Follicle-Stimulating Hormone (FSH).
  3. Testicles (Leydig Cells): LH travels through the bloodstream to the testicles, where it signals the Leydig cells to produce and secrete testosterone.
  4. Feedback Loop: As testosterone levels in the blood rise, this signals back to the hypothalamus and pituitary to decrease the release of GnRH and LH, thus keeping testosterone within a normal physiological range. This is a classic negative feedback system.

Hypogonadism can occur if there is a problem at any point in this axis.

The Link Between Obesity and Low Testosterone: A Vicious Cycle

One of the strongest clinical correlations I see in my practice is the link between obesity and low testosterone. Over 50% of heavier men exhibit low testosterone levels. However, this presents a diagnostic puzzle. When you send these men for a standard total testosterone blood test, the result often comes back in the “normal” range. This can be confusing for both the patient and the provider.

Here is the key to understanding this paradox: obesity affects a protein called Sex Hormone-Binding Globulin (SHBG). SHBG, as its name implies, binds tightly to testosterone in the bloodstream, rendering it inactive. Only the “free” or unbound testosterone is biologically active and able to exert its effects on target tissues.

Heavier individuals, particularly those with insulin resistance, tend to have lower levels of SHBG. With less SHBG available to bind testosterone, a higher percentage of their total testosterone is in the free, active form. This can make their total testosterone level appear artificially normal, even when the overall production is insufficient. This is why for any man, and especially for a heavier man, it is absolutely critical to measure not just total testosterone but also free testosterone and SHBG. This provides a much more accurate picture of their true hormonal status.

The relationship is a vicious cycle: low testosterone promotes fat storage (especially visceral abdominal fat), and the excess body fat (specifically adipose tissue) contains an enzyme called aromatase, which converts testosterone into estrogen. This further lowers testosterone and raises estrogen, worsening the hormonal imbalance and making it even harder to lose weight.

Other Health Implications of Low Testosterone

The impact of low testosterone extends far beyond sexual function and body composition. Evidence-based research has linked low T to a host of other chronic health conditions:

  • Type 2 Diabetes: Higher testosterone levels are associated with a lower risk of developing diabetes, as testosterone improves insulin sensitivity.
  • Sleep Apnea: This is a critical contraindication to be aware of. While low T is associated with conditions that cause sleep apnea (like obesity), starting testosterone replacement therapy (TRT) can actually worsen existing sleep apnea. Testosterone can relax the musculature in the upper airway, increasing the risk of collapse during sleep. Therefore, it is imperative to screen for and address sleep apnea before initiating TRT, especially in heavier patients.
  • Hypertension and Hyperlipidemia: Men with low T are more likely to have high blood pressure and unhealthy cholesterol profiles.
  • COPD and Kidney Disease: These chronic conditions are also associated with lower testosterone levels.
  • Rheumatoid Arthritis (RA): Intriguingly, low testosterone has been shown to be an independent risk factor for developing RA in men.

Diagnosing Hypogonadism: The Right Way to Test

Diagnosing hypogonadism requires a careful and methodical approach. It is not as simple as looking at a single number on a lab report.

The Official Definition

According to the American Urological Association (AUA) guidelines, the biochemical diagnosis of hypogonadism is typically made when a man has a total testosterone level of less than 300 nanograms per deciliter (ng/dL). However, this is not a hard-and-fast rule. The most important part of the diagnosis is that the patient must also have associated signs and symptoms. I never treat just a number. If a man has a testosterone level of 250 ng/dL but feels fantastic and has no symptoms, he does not require treatment. Conversely, a man with a level of 350 ng/dL who is suffering from severe symptoms may well be a candidate for therapy.

Proper Lab Testing Protocol

To get an accurate reading, it is crucial to follow a specific protocol:

  1. Morning Draw: Testosterone levels peak in the early morning. Therefore, blood for testosterone testing must always be drawn first thing in the morning, typically between 7 AM and 10 AM.
  2. Confirm with a Second Test: A single low reading is not sufficient for a diagnosis. The AUA recommends obtaining at least two separate morning samples and using the lower of the two values as the diagnostic parameter. This helps rule out transient fluctuations.
  3. Investigate the HPTA Axis: Once low testosterone is confirmed, the next step is to determine why it is low. Is it a problem with the testicles (primary hypogonadism) or a problem with the brain signals (secondary hypogonadism)? To figure this out, we must check the Luteinizing Hormone (LH) level.
    • Low Testosterone + High LH: This pattern indicates primary hypogonadism. The pituitary is screaming at the testicles to make testosterone (high LH), but they are failing to respond. This can be caused by aging, trauma, infection, or genetic conditions.
    • Low Testosterone + Low or Normal LH: This pattern indicates secondary hypogonadism. The testicles are capable of making testosterone, but they are not getting the signal from the pituitary. The brain is failing to send the “go” signal.
  4. Rule Out Pituitary Tumors: If you find a pattern of secondary hypogonadism (low T, low LH), it is essential to check a prolactin level. An elevated prolactin level can be a sign of a prolactinoma, a benign tumor of the pituitary gland that can suppress the entire HPTA axis. These are more common than most people think and are a treatable cause of low testosterone.
  5. Check Estradiol and SHBG: As mentioned, checking SHBG and free testosterone is vital, especially in heavier men. We also need to check estradiol (estrogen), particularly if a man presents with gynecomastia (breast tissue development). As men age and testosterone levels drop, the enzyme aromatase can become more active, converting the remaining testosterone into estrogen. The testosterone-to-estradiol ratio is a key metric. A healthy ratio is around 20:1. When this ratio drops to 15:1 or 10:1, men will definitely start to experience symptoms of estrogen dominance, such as moodiness, water retention, and fat gain.

Validated Screening Tools for Clinical Use

To help systematically identify patients who may be suffering from low testosterone, we use validated questionnaires in our clinic. These are invaluable tools for starting the conversation.

  • The Androgen Deficiency in Aging Males (ADAM) Questionnaire: This is a simple 10-question screen. The two most important questions are: “Do you have a decrease in libido (sex drive)?” and “Are your erections less strong?” Answering “yes” to these and/or several other questions has an 88% sensitivity for detecting low testosterone.
  • The Sexual Health Inventory for Males (SHIM): This questionnaire is more specific to sexual function. It has a 100% specificity, meaning that a low score is a very strong indicator of an underlying problem, often linked to low testosterone.

These are tools that any practitioner can easily incorporate into their patient intake process to screen for this common but underdiagnosed condition.

Testosterone Replacement Therapy (TRT): Goals, Guidelines, and Safety

Once a proper diagnosis of symptomatic hypogonadism has been made, the conversation can turn to treatment with Testosterone Replacement Therapy (TRT).

The Goals of TRT

The primary goal of TRT is not to treat a number on a lab report. The primary goal is to reduce symptoms and improve the patient’s quality of life. The benefits we aim for include:

  • Improved energy, mood, and cognitive function
  • Increased libido and improved sexual function
  • Increased lean muscle mass and decreased body fat
  • Improved bone mineral density
  • Better glycemic control in diabetic patients

The therapeutic target range for total testosterone is typically between 450 and 600 ng/dL. I call this the “utopian” range. In clinical reality, it can be challenging to dial in the dose perfectly, and levels may fluctuate. It is common to see levels rise to 800 or 900 ng/dL. The key is to avoid supraphysiological levels (well over 1,000 ng/dL) and, most importantly, to monitor the patient’s symptoms and side effects.

AUA Guidelines for TRT

Before starting TRT, it is crucial to perform a thorough history and physical exam to ensure the patient’s symptoms are not coming from another underlying condition. We must also consider several important contraindications and cautions.

  • Prostate Cancer: Historically, there was a great deal of concern that TRT could cause or worsen prostate cancer. Decades of research have largely debunked this idea. The current AUA guidelines state that TRT can be considered for men with a history of prostate cancer who have completed their treatment and have no evidence of active disease. The old “five-year window” has been shortened. It is now considered reasonably safe to start TRT as early as one to two years after definitive treatment (like a radical prostatectomy), provided the patient is closely monitored with regular PSA tests and digital rectal exams.
  • Cardiovascular Events: TRT should not be initiated within three to six months of a major cardiovascular event, such as a heart attack or stroke. In these cases, I always pick up the phone and have a direct conversation with the patient’s cardiologist to ensure we are all on the same page and that initiating therapy is deemed safe.
  • When to Stop Therapy: The old stigma that testosterone therapy must be stopped once a man reaches a certain age is also gone. The AUA now states that as long as a patient continues to benefit from TRT, has no contraindications, and is being properly monitored, therapy can be continued well into their 80s and 90s.

The Role of TRT in Treating ED

It is important to be clear: TRT is not a primary, first-line treatment for erectile dysfunction. The most common cause of ED is vascular, not hormonal. However, optimizing testosterone is a critical adjunctive therapy. Restoring testosterone to a healthy range will almost certainly improve libido, which is the “desire” component of sexual function. It may have a modest direct effect on erections, but its most powerful role is in enhancing the response to other ED therapies. For men who have developed tachyphylaxis to PDE5 inhibitors like Viagra or Cialis, getting their testosterone levels into the optimal range can often restore the effectiveness of those medications.

Monitoring and Managing Risks of TRT

Like any medical therapy, TRT is not without risks and requires diligent monitoring.

  1. Erythrocytosis: This is the most common and clinically significant side effect. Testosterone stimulates the bone marrow to produce red blood cells. In some men, this can lead to an abnormally high red blood cell count or hematocrit, a condition called erythrocytosis or polycythemia. This thickens the blood and increases the risk of blood clots, stroke, and heart attack. It is absolutely mandatory to monitor a patient’s complete blood count (CBC) and hematocrit regularly. If the hematocrit rises to a dangerous level (typically >54%), the TRT dose must be lowered or temporarily stopped.
  2. Prostate Monitoring: While TRT does not cause prostate cancer, it can potentially stimulate the growth of a pre-existing, undiagnosed cancer. Therefore, regular monitoring with Prostate-Specific Antigen (PSA) blood tests and digital rectal exams is required.
  3. Other Side Effects: Other potential side effects include acne or oily skin, fluid retention, worsening of lower urinary tract symptoms (BPH), and gynecomastia (if testosterone is excessively converted to estrogen).

Our protocol for managing a patient with ED and suspected low T is systematic:

  1. Evaluate the ED and perform a full history and physical.
  2. Check morning testosterone levels (total, free, SHBG) and other relevant hormones (LH, estradiol, prolactin).
  3. Confirm the diagnosis of symptomatic hypogonadism.
  4. Initiate TRT if indicated and safe to do so.
  5. Consider adding or continuing PDE5 inhibitors, as there is no contraindication.
  6. Monitor PSA, hematocrit, and hormone levels regularly.
  7. Most importantly, follow up with the patient to ensure their symptoms are improving and they are not experiencing adverse effects.

The Cutting Edge: Peptide Therapies for Men’s Health

With a solid hormonal foundation in place, we can now explore the exciting world of peptide therapies. Peptides are short chains of amino acids that act as signaling molecules in the body, telling cells and tissues what to do. They offer a highly specific and targeted way to influence physiological processes, including sexual function.

Gonadotropin-Based Peptides for Natural Testosterone Production

For men who wish to boost their testosterone levels without shutting down their body’s own production (a consequence of direct testosterone replacement), gonadotropin-based peptides are an excellent option.

  • Human Chorionic Gonadotropin (hCG): hCG is a peptide that mimics the action of Luteinizing Hormone (LH). By stimulating the Leydig cells in the testicles directly, hCG can “jump-start” endogenous (natural) testosterone production. It is particularly useful for men with secondary hypogonadism (low LH). It is also the standard of care for restoring fertility in men who have been on long-term TRT, as direct testosterone administration suppresses both LH and FSH, leading to testicular shrinkage and a halt in sperm production. Potential side effects can include an increase in estradiol, oily skin, and mood swings if not dosed correctly.
  • Gonadorelin: This is a synthetic version of Gonadotropin-Releasing Hormone (GnRH). By mimicking the natural pulsatile release of GnRH from the hypothalamus, gonadorelin stimulates the pituitary to produce its own LH and FSH. This provides a more “upstream” and natural way to boost the entire HPTA axis. The increase in FSH also stimulates the Sertoli cells in the testicles, which can increase sperm counts, making it another excellent choice for men concerned about fertility.

Peptides for Libido and Erectile Function

Beyond boosting testosterone, several peptides act directly on the pathways involved in sexual arousal and function.

  • PT-141 (Bremelanotide): This is one of the most well-known peptides for sexual health. It is FDA-approved for hypoactive sexual desire disorder in women and is used off-label for men. Unlike PDE5 inhibitors, which act on the vascular system, PT-141 acts centrally in the brain on melanocortin receptors. It acts independently of the nitric oxide pathway to directly increase libido and sexual desire, and it can also improve erectile function as a downstream effect of heightened arousal. The primary challenge with PT-141 is the significant risk of nausea, which affects up to 40% of users, especially women. The key is to start with a very low dose and titrate up slowly.
  • BPC-157: Known as “Body Protective Compound,” BPC-157 is a remarkable peptide renowned for its systemic healing and regenerative properties. While not a direct sexual enhancement peptide, it contributes to erectile health in several important ways. It promotes angiogenesis (new blood vessel growth) and powerfully modulates nitric oxide signaling, which, as we have extensively discussed, is critical for erections (Seiwerth et al., 2021).
  • TB-500 (Thymosin Beta-4): Often used in combination with BPC-157 in what is sometimes called the “Wolverine Stack,” TB-500 is another potent regenerative peptide. I like to think of BPC-157 as the “foreman” that brings all the healing materials to the job site, while TB-500 is the “master engineer” that orchestrates the assembly and repair process. Together, they promote the regeneration of any damaged tissue in the body, including the vascular and neural tissues of the penis.

Combination Peptide Protocols

The true power of peptides lies in creating intelligent, synergistic combinations to address multiple aspects of a patient’s health.

  • For Anti-Aging and Sexual Vitality: A combination of CJC-1295 (a growth hormone-releasing hormone analog) and Ipamorelin (a growth hormone secretagogue) works synergistically to stimulate the body’s natural, pulsatile release of growth hormone. This can lead to improved sleep quality, fat loss, enhanced recovery, and a general increase in vitality and sexual well-being.
  • For Libido and Erection Quality: Combining PT-141 (for central libido enhancement) with a low-dose daily PDE5 inhibitor like tadalafil can be a potent combination, addressing both the “desire” and the “mechanics” of sexual function.
  • For Testosterone Support Without Fertility Suppression: For men who need testosterone support but wish to maintain fertility, a combination of low-dose TRT and hCG is a common and effective strategy. The TRT provides a stable baseline of testosterone, while the hCG keeps the testicles stimulated and sperm production online.

The Integrated Chiropractic Role in Men’s Health

As a Doctor of Chiropractic, my primary focus is on the intricate relationship between the body’s structure—particularly the spine—and the function of the nervous system. While it might not seem immediately obvious, chiropractic care plays a vital and complementary role in the comprehensive management of men’s health issues, including ED.

The nerve supply that controls erectile function originates in the sacral plexus, a complex network of nerves in the lower back and pelvic region (S2-S4 spinal segments). Misalignments or subluxations in the lumbar spine and sacroiliac joints can create biomechanical stress and neurological interference that can directly impact the function of these crucial nerves.

By performing specific chiropractic adjustments, we can:

  • Restore Proper Pelvic Biomechanics: Correcting misalignments in the lumbar spine and pelvis can alleviate pressure on the sacral nerve roots.

By properly diagnosing and treating low testosterone, we can rebuild the hormonal foundation of male vitality. And with advanced peptide therapies, we can fine-tune the body’s own signaling systems to optimize function.

By integrating these cutting-edge modalities with the foundational principles of chiropractic care and the expert oversight of internal medicine, we can offer our patients a truly holistic path back to health.
