Patients don’t wake up one morning hoping for a PRP (Platelet-Rich Plasma) injection or any other specific medical procedure. No one ever thought, “You know what would be great today? A rotator cuff tear, so I can’t play pickleball or lift my grandkids.” People don’t seek out pain or limitations. They seek recovery, function, and the ability to return to the life they love.

The patient’s journey to our clinic begins with a desire to shed the limitations imposed by their condition. It’s a common misconception to think we are simply selling a procedure. We’re committed to a better, healthier future. True value lies in life transformation. Consider the common scenario where a surgeon tells a patient, “I can’t operate on you until you lose weight.” While the advice is sound from a surgical-risk perspective, it’s often delivered without adequate support.

This is where our integrative model shines. We can say:

  • “One of the most effective treatments for your knee arthritis is weight loss. Did you know that losing just ten pounds can reduce the pressure on your knees by thirty pounds?”
  • “Would you like help with that? We have a program designed to guide you. Over the next eight weeks, our patients typically lose around twelve pounds.”
  • “We see this success in 95% of patients who commit to this program.”

This approach isn’t just helpful; it’s massively valuable. We are not just identifying a problem; we are providing the solution and the support system to achieve it.

The Current State of Orthobiologics: Science vs. Execution

Throughout my years in practice and my continuous study of regenerative medicine, I’ve observed a recurring theme. When orthobiologic treatments fail to deliver on their promise, it is rarely an issue with the underlying science. The failure is almost always in the execution.

Many of us in this field believe that orthobiologics should be a first-line treatment for a vast array of musculoskeletal (MSK) conditions. The primary hurdle, however, is that these services are typically cash-based. This is where poor execution can derail an otherwise effective therapy.

  • Inconsistent Outcomes: Clinic A’s protocol can differ markedly from Clinic B’s. This lack of standardization leads to unpredictable results and erodes patient trust.
  • Poor Patient Selection: Is Platelet-Rich Plasma (PRP) a miracle cure for everything? While it is remarkably versatile, it is not a panacea. Offering PRP for severe, bone-on-bone knee arthritis or hip arthritis with significant bone marrow edema is an example of poor patient selection.
  • Overpromising in Marketing: We live in an era I call the “wild, wild west” of regenerative medicine. Direct-to-consumer marketing often overpromises and underdelivers, ultimately eroding public trust. My philosophy is the opposite: under-promise and over-deliver.
  • Lack of Standardization and Data: Are we measuring what we are injecting? We now have strong evidence suggesting that a PRP dose exceeding 5 billion platelets yields significantly better clinical outcomes. Every practitioner should be measuring platelet counts for every patient, every time. If you cannot measure, the next best thing is to know your system’s output. For example, we’ve tested the Apex kit extensively and know that a 60cc blood draw reliably yields approximately 10.8 billion platelets.

I believe we are at an inflection point. If we commit to doing the work—collecting data, using ultrasound guidance, and meticulously selecting patients—orthobiologics will become a standard first-line treatment within the next five years.

To address these execution challenges, I developed a framework I call the Joint Vitality System. It’s based on the philosophy that regenerative medicine is not just a procedure; it is a comprehensive system of care.

  1. Precision Diagnosis: Achieving an accurate diagnosis is the non-negotiable first step.
  2. Biologic Matching: Thoughtfully selecting the appropriate biologic agent for the specific tissue pathology.

  4. Guided Recovery: Implementing a comprehensive rehabilitation program to ensure long-term success.

Precision Diagnosis: More Than Just an MRI

I often call myself a “show-me state” practitioner. I don’t believe it until I can prove it. This means using every tool at my disposal to understand not only what is wrong but why it failed.

  • Physical Exam and History: It starts with the fundamentals: a thorough history and a hands-on physical exam.
  • Diagnostic Ultrasound: This tool is not optional in my practice. Real-time imaging allows me to pinpoint the exact source of pain by correlating the patient’s sensation with the visualized anatomy.
  • Advanced Imaging (MRI): I get MRIs for almost everyone to evaluate deeper structures such as cartilage and bone. However, I never rely on the report alone. I review the films myself, often with a trusted MSK radiologist. Standard reports often miss subtle yet clinically significant findings that are highly responsive to orthobiologic treatments.
  • Diagnostic Injections: One of the most powerful tools for confirming a diagnosis is a diagnostic injection. By numbing a specific structure, we can determine if it is the primary pain generator. When the patient experiences significant relief, it builds immense confidence for a cash-based regenerative procedure.

Understanding the “Why”: Uncovering the Root Cause

A precise diagnosis also requires understanding the underlying driver of the pathology. Is the pain driven by inflammation, degeneration, a structural problem, or a neurological issue? We must also consider the patient’s systemic biology. I routinely test my regenerative medicine patients for thyroid function, Hemoglobin A1c, and sex hormones. We know that arthritis develops, on average, 20 years sooner in women than in men, which correlates with the decline in estrogen during menopause. Similarly, a patient with a Hemoglobin A1c of 10% will have a severely impaired healing response. We must address these systemic factors to optimize the outcome of any local orthobiologic treatment.

  • Degeneration and Cell Signaling: Degenerative conditions often involve a deficiency of stem cells. Using BMC intraosseously can reintroduce necessary cells and growth factors to a “biologic dead zone,” enhancing healing.

The procedure itself—whether PRP or another orthobiologic treatment—is a pivotal moment. But it’s Day One, not the final step. This intervention acts as a powerful catalyst, signaling the body to initiate a robust healing cascade.

The post-procedure period is where the real work of transformation continues. Our job is not done once the injection is administered. This guided recovery phase includes personalized rehabilitation, lifestyle coaching, and regular follow-ups. As a Doctor of Chiropractic, my primary focus is on the body’s structure and its relationship to nervous system function and overall health. In our integrative model, chiropractic care is a vital component woven throughout the recovery process.

These figures represent a sustainable business that allows you to practice medicine on your own terms. For example, in my practice, we dedicate an entire day to just four procedure patients. Can you imagine the level of attention and care those individuals receive?

Debunking the Myths of Running a Medical Business

Many of us in the medical field believe that business is more difficult or intimidating than medicine. I want to assure you it is not. You don’t need an MBA to start a small, specialized medical practice. The risks in business are primarily financial, not physical. You don’t need to have every single step figured out before you start. You need a few key elements: a sense of humor, a way to handle phone calls, a system for record-keeping, and the necessary clinical tools. For an orthobiologics practice, the most significant investment is often an ultrasound machine, which is crucial for diagnostic accuracy and procedural guidance.

Building Your Practice Through Relationships, Not Marketing

The most durable and effective way to grow an orthobiologics practice is through relationships.

  1. Tap Into Your Existing Patient Base: The patients who already know, like, and trust you are your most valuable asset. The cost of acquiring a new patient is substantial, but the cost of engaging an existing one is zero.
  2. Cultivate a Strong Referral Network: Building relationships with other providers (PTs, PCPs, Chiropractors, even Orthopedic Surgeons) creates a steady stream of ideal patients. Approximately 75% of the patients seen in an orthopedic surgery office are non-surgical candidates. By positioning yourself as the go-to solution for these problems, you become an invaluable resource.
  3. The Power of Word-of-Mouth: When you deliver transformative care, your patients become your greatest advocates. This organic marketing is more powerful than any paid advertisement.

The Power of Data: Proving What Works

In the rapidly advancing field of orthobiologics, a significant challenge is data loss. Many great clinicians are not systematically collecting data on their outcomes. As a solo practitioner, implementing a data collection system was a deliberate choice—and an investment.

Why is data so important?

  1. To Validate Our Treatments: It allows us to prove, with objective evidence, that what we are doing works.
  2. To Refine Our Protocols: By tracking outcomes, we can continuously improve our approach.
  3. To Empower Patients: When we can show a new patient data from past patients with similar conditions, it builds confidence and trust.

We use a system to track key metrics, including Pain and Function Scores and Patient-Reported Outcomes (PROs). We are transparent about the cost, adding a nominal fee of about $25 to the treatment package. Virtually every patient agrees. They want their doctor to be collecting data.

Your Identity Shift: From Technician to Healer

Adopting the Precision Practice model requires an identity shift. It means broadening our perspective to encompass the full spectrum of a person’s health—their biochemistry, biomechanics, lifestyle, and goals. We demonstrated these principles with a live patient—my 80-year-old father, an avid pickleball player who injured his shoulder.
