Why Injuries Need More Than Symptom Control

Layer One: Structural and Mechanical Care

Structural care focuses on how the spine, joints, muscles, and nerves work together. Chiropractic adjustments, joint mobilization, soft-tissue care, and selected forms of spinal decompression may reduce stiffness and improve movement.

Research suggests that spinal manipulative therapy may provide small improvements for some people with chronic low-back pain. It is not a guaranteed cure and may work about as well as other conservative treatments. Results are often better when hands-on care combines with exercise, education, and gradual activity.

An updated Cochrane review examined 76 studies involving 11,866 people. It found that spinal manipulation may improve pain and function compared with no treatment, but the benefits were smaller than those of other conservative treatments. The researchers also reported low confidence in some of the evidence due to variation in treatment methods and study quality (Cochrane, 2026).

Spinal decompression or traction may be considered for selected patients with disc-related pain or nerve symptoms. These treatments use controlled pulling forces to change pressure and improve movement tolerance. However, not every bulging disc causes pain, and decompression does not place every disc “back into position.” Care should be based on symptoms, examination findings, and imaging when needed.

Structural care may help by:

  • Improving joint motion
  • Reducing protective muscle tension
  • Making movement less painful
  • Supporting better body mechanics
  • Preparing the patient for exercise

Layer Two: Biological and Regenerative Support

PRP preparation also matters. Platelet concentration, white blood cell content, injection location, and the patient’s health may affect the response. Micro-fragmented adipose tissue, or MFAT, is collected from the patient’s own fat and processed for certain orthopedic procedures. It contains structural tissue, signaling molecules, and cell populations that may support the local tissue environment.

Current evidence is stronger for knee osteoarthritis than for routine spinal treatment. Reviews report improvements in pain and function for some knee patients, but outcomes vary. PRP may also provide equal or better long-term pain relief in certain comparisons (Hohmann et al., 2025; Ye et al., 2024).

PRP, bone marrow aspirate concentrate, and MFAT come from different sources and are not interchangeable. The best choice depends on the diagnosis, injury location, procedure risks, cost, available evidence, and the patient’s overall health (Sports Medicine of the Rockies, 2026).

Adipose-derived products are also regulated. Patients should ask what product is being used, how it is processed, and what evidence supports the proposed use. The U.S. Food and Drug Administration warns that many marketed stem-cell, exosome, and tissue products have not been approved for the treatment of orthopedic conditions such as disc disease, arthritis, back pain, neck pain, or tendon injuries (U.S. Food and Drug Administration, 2021, 2026).

Laser and Shockwave Therapy

Photobiomodulation, often called therapeutic laser treatment, uses red or near-infrared light. The light interacts with cells and may affect cellular energy production, inflammation, and pain signaling. Results depend on the device, dose, injury, and treatment schedule (Harrington, n.d.).

Shockwave therapy sends acoustic energy into an injured area. It is often used for chronic tendon problems and painful scar-like tissue. It may help produce a local healing response in selected conditions, but it is not right for every injury.

Laser and shockwave therapy should support a larger rehabilitation plan. They should not replace a correct diagnosis, movement training, or medical referral when serious tissue damage is present.

Layer Three: Nutritional and Systemic Support

Healing requires energy, protein, vitamins, minerals, hydration, sleep, and stable blood sugar. A person who is undernourished, dehydrated, highly stressed, or sleeping poorly may have a harder time completing rehabilitation.

A practical nutrition plan may include:

  • Enough protein for muscle and connective tissue
  • Fruits and vegetables
  • Healthy fats, including omega-3 sources
  • Water and needed electrolytes
  • Fiber-rich foods
  • Fewer ultra-processed foods and added sugars

IV nutrition delivers fluids or nutrients directly into the bloodstream. It bypasses the digestive tract, but this does not mean every nutrient is needed or fully used by the body. Evidence for the use of commercial IV vitamin infusions in otherwise healthy people remains limited.

IV treatment may also cause bruising, vein irritation, infection, fluid problems, medication interactions, or complications from excessive nutrient doses. High-dose vitamin C, for example, may be unsafe for some patients with kidney problems or glucose-6-phosphate dehydrogenase deficiency (Alangari, 2025).

IV nutrition should be medically screened and used for a clear reason, such as dehydration, a confirmed deficiency, poor absorption, or another suitable medical need. Kidney function, heart health, medications, allergies, and laboratory results should be reviewed first. Follow-up is also important so that the care plan can be adjusted based on the patient’s response.

Passive treatments may reduce pain, but rehabilitation teaches the body how to move again. Patients may need mobility exercises, core control, balance work, endurance training, and progressive resistance exercises.
