Sciatica is not just “leg pain.” It is usually a sign that the sciatic nerve or one of its nerve roots is irritated, inflamed, compressed, or chemically aggravated. Common causes include:

  • Herniated or bulging discs
  • Inflamed spinal joints
  • Ligament injury or instability
  • Degenerative disc changes
  • Scar tissue after old injuries
  • Spinal stenosis
  • Personal injury trauma from auto, work, or sports accidents

Traditional care often focuses on calming pain. Regenerative and integrative care goes further. The goal is to reduce inflammation, improve joint mechanics, support damaged spinal tissues, and help the body heal more completely.

Why Sciatica Happens

The sciatic nerve is the largest nerve in the body. It starts from nerve roots in the lower spine, travels through the pelvis, and runs down the back of the leg. When the nerve root is pinched or inflamed, pain may travel far from the spine.

PRP: Platelet-Rich Plasma for Sciatica

PRP stands for platelet-rich plasma. It is made from a patient’s own blood. A small blood sample is drawn and centrifuged. This separates the blood into layers and concentrates the platelets.

Platelets are best known for helping blood clot, but they also carry growth factors and signaling proteins. These growth factors may help calm inflammation and support tissue repair (Ohio State Wexner Medical Center, n.d.).

For sciatica, PRP may be used near damaged spinal structures or as part of a specialized epidural approach. Research suggests epidural PRP may offer results similar to epidural steroid injections for lumbar disc-related radiculopathy, with a comparable safety profile, though more large studies are still needed (Muthu et al., 2025).

PRP may help by:

  • Reducing local inflammation around irritated nerves
  • Supporting healing of injured soft tissue
  • Helping improve pain and function over time
  • Offering a non-steroidal option for selected patients
  • Using the patient’s own blood, which lowers allergy risk

PRP is not instant pain relief for everyone. Steroids may work faster in the first days or weeks. PRP is often described as a slower biologic approach that may continue to work over weeks to months.

PFP: Platelet-Fibrin Products as a Healing Scaffold

PFP stands for platelet-fibrin products. These products are related to platelet therapy but include a fibrin matrix. Fibrin is a natural protein involved in clotting and tissue repair.

A helpful way to understand PFP is to think of it as a natural scaffold. Instead of releasing all growth factors quickly, the fibrin structure may help hold healing signals in place and release them over time.

For spinal ligaments, tendons, and disc-related tissues, this may be useful because these areas often need longer support. PFP may help create a local environment that supports repair, stability, and tissue remodeling.

In simple terms:

  • PRP delivers concentrated platelets and growth factors.
  • PFP adds a fibrin framework that may support longer release.
  • Both are biologic tools used to support the body’s healing response.

These treatments are not a cure-all. They require a correct diagnosis, imaging when needed, and careful medical decision-making.

mFAT: Microfragmented Adipose Tissue

mFAT stands for microfragmented adipose tissue. Adipose tissue is fat tissue. During an mFAT procedure, a small amount of fat is collected from the patient, gently processed, and injected into the injured area.

Fat tissue contains biologic signaling cells, growth factors, and anti-inflammatory molecules. It may also provide cushioning and support in damaged tissues (University of Iowa Health Care, n.d.).

mFAT is often discussed for more advanced joint, tendon, or degenerative conditions. It may be considered when tissue damage is more complex or when PRP alone may not be enough.

Potential goals of mFAT include:

  • Helping regulate chronic inflammation
  • Supporting tissue repair
  • Improving the local joint or soft tissue environment
  • Providing biologic cushioning
  • Helping patients recover function over time

Like PRP, mFAT is not usually an instant fix. Healing is gradual. Patients may need rest, guided rehab, and follow-up care to get the best results.

Traditional Epidural Injections for Sciatica

Epidural steroid injections are one of the most common procedures for sciatica and lumbar radiculopathy. In this procedure, medication is placed into the epidural space around irritated spinal nerves.

A traditional epidural often includes:

  • A corticosteroid to reduce inflammation
  • A local anesthetic to reduce pain

This can help many patients get fast relief. Pain may improve over weeks or months, making it easier to walk, sleep, work, and participate in rehabilitation (DISC Sports & Spine Center, n.d.).

However, traditional epidurals usually focus on symptom control. They may reduce nerve swelling, but they do not usually repair a torn disc, weak ligament, or damaged joint structure. If the underlying problem remains, symptoms can return.

That does not mean steroid epidurals are bad. They can be very helpful for the right patient, especially when pain is severe. The key is to understand their role. They are often a bridge to rehab, not the whole treatment plan.

Regenerative Epidurals: Platelet Lysate and PRP-Based Options

Regenerative epidurals use biologic materials instead of, or sometimes alongside, corticosteroids. One example is platelet lysate, a processed form of PRP. Platelet lysate is produced by lysing platelets to release growth factors and anti-inflammatory proteins.

A large case series on lumbar epidural platelet lysate reported significant improvements in pain and function at follow-up, with no serious adverse events. The authors described platelet lysate as a promising alternative to corticosteroids in selected cases of lumbar radicular pain (Centeno et al., 2017).

A systematic review also found that PRP and platelet-related epidural products were associated with improved pain and function across the reviewed studies, but it rated the certainty of the evidence as very low due to study limitations, risk of bias, and small sample sizes (Kubrova et al., 2022).

This means regenerative epidurals are promising, but they should be presented honestly. They may help selected patients, but they are still developing in the research world.

Why Integrative Chiropractic Care Matters

Sciatica is not only a chemical inflammation problem. It is also often a mechanical problem.

If spinal joints are not moving well, muscles are guarding, the pelvis is rotated, or posture has changed after injury, the sciatic nerve can stay irritated. This is where chiropractic care can be an important part of the plan.
