Car Accident Back Pain: Relief through Chiropractic and Epidural

Car Accident Back Pain: Epidural and Chiropractic Care

Abstract

Back injuries from motor vehicle accidents can involve more than sore muscles. A crash may irritate spinal nerves, injure a disc, limit joint movement, and cause the muscles to tighten around the injured area. For selected patients, combining an epidural spinal injection with integrative chiropractic care may address different parts of the injury. The injection may temporarily reduce inflammation around an irritated spinal nerve. Chiropractic care and rehabilitation may then help restore joint movement, reduce muscle tension, correct harmful movement patterns, and rebuild strength. This article explains how these treatments may work together, which patients may benefit, why medical oversight matters, and how a multidisciplinary team can guide recovery. It also discusses the clinical approach used by Dr. Alexander Jimenez, DC, APRN, FNP-BC, CCST, CFMP, IFMCP, ATN, and Dr. Maria Guadalupe Cardenas, MD, at Injury Medical Clinic PA in El Paso, Texas.

Car Accident Back Pain: Relief through Chiropractic and Epidural

How a Motor Vehicle Accident Can Injure the Back

A motor vehicle accident places sudden force through the spine. Even when a collision occurs at a lower speed, the body may move forward, backward, or sideways before the muscles have time to protect the joints. This sudden movement can affect the neck, middle back, lower back, shoulders, hips, and pelvis.

Common accident-related injuries may include:

  • Muscle strains
  • Ligament sprains
  • Whiplash
  • Herniated or bulging spinal discs
  • Irritated spinal nerves
  • Joint stiffness
  • Reduced range of motion
  • Muscle guarding and spasms
  • Sciatica or radiating leg pain
  • Changes in posture and walking patterns

Pain may not always appear right away. Stress hormones released during the crash can temporarily hide discomfort. As the stress response settles, stiffness, headaches, back pain, numbness, tingling, or weakness may become more noticeable. A complete examination is important because accident injuries may involve both irritated nerves and mechanical problems in the joints and muscles (Back Pain Center, 2026; In8 Wellness, 2024).

What Is an Epidural Spinal Injection?

An epidural spinal injection places medication into the epidural space near an irritated spinal nerve. The injection often contains a corticosteroid medication and, in some cases, a local anesthetic. The anesthetic may provide faster temporary relief, while the corticosteroid works over the following days to decrease inflammation around the nerve.

Epidural injections are most often considered when pain travels along a nerve. Examples include pain that moves from the lower back into the buttock or leg, or pain that travels from the neck into the shoulder, arm, or hand. These symptoms may occur when a herniated disc or another spinal condition irritates a nerve root (Fanaee, n.d.; Houston Pain Specialists, n.d.).

Research suggests that epidural steroid injections may provide limited short-term improvement in pain and disability for some people with cervical or lumbar radiculopathy. However, the results vary, and the injections do not consistently provide long-term relief or prevent surgery. They should not be presented as a permanent cure for every type of back pain (Armon et al., 2025; Oliveira et al., 2020).

How Integrative Chiropractic Care Fits Into Recovery

An epidural injection mainly targets nerve irritation and inflammation. It does not automatically restore normal joint motion, correct muscle weakness, or improve the way a person bends, walks, lifts, or sits.

Integrative chiropractic care focuses on these mechanical and functional parts of recovery. Depending on the diagnosis and stage of healing, care may include:

  • Gentle spinal or joint mobilization
  • Chiropractic adjustments when medically appropriate
  • Soft-tissue therapy
  • Myofascial release
  • Stretching
  • Posture correction
  • Core stabilization
  • Corrective exercises
  • Balance and coordination training
  • Home-care education
  • Gradual return-to-work or activity planning

Manual care should be adjusted to the patient’s condition. A person with a possible fracture, severe neurological loss, spinal instability, infection, or another serious condition requires medical evaluation rather than routine manipulation.

Evidence for whiplash and related neck injuries supports a multimodal approach that may combine manual therapy, exercise, education, and activity guidance. No single treatment works for every patient. Care is more useful when it is matched to the person’s symptoms, physical findings, injury stage, and recovery goals (Côté et al., 2016; Sutton et al., 2016).

Potential Benefits of Combining Both Treatments

There is limited direct research comparing the exact combination of epidural injections and chiropractic care with either treatment used alone. Therefore, the possible benefits are based on how the two treatments address different parts of the injury.

1. Reduced Nerve Inflammation May Make Movement Easier

Severe nerve pain can make it difficult for a patient to walk, stretch, exercise, or tolerate hands-on treatment. When an epidural injection reduces nerve irritation, the patient may be able to participate more comfortably in chiropractic care and rehabilitation.

Houston Pain Specialists notes that relief after an epidural injection may make physical therapy and chiropractic therapy easier to perform. The goal is not simply to cover the pain. It is to create a temporary period in which the patient can move, exercise, and rebuild function with less discomfort (Houston Pain Specialists, n.d.).

2. Chiropractic Care May Improve Joint Movement

After a crash, injured joints may become stiff. The body may protect the painful area by tightening nearby muscles and changing the way the patient moves. These changes can place extra stress on the hips, pelvis, shoulders, knees, or opposite side of the spine.

Chiropractic care may help restore appropriate joint movement and reduce physical tension. Soft-tissue techniques may also help decrease muscle guarding. Exercises can then be used to improve strength, flexibility, balance, and control (Back Pain Center, 2026; In8 Wellness, 2024).

3. The Combination May Address Pain and Function

An injection may reduce inflammation around an irritated nerve, but pain relief alone does not prove that the injured area has regained normal strength or movement. Chiropractic care and rehabilitation can focus on the physical problems that remain after the pain decreases.

A coordinated plan may address:

  • Nerve inflammation
  • Joint stiffness
  • Muscle tightness
  • Weakness
  • Poor posture
  • Reduced flexibility
  • Abnormal walking or lifting patterns
  • Fear of movement
  • Difficulty returning to work

Dr. Jimenez’s published clinical observations describe this as treating both sides of an injury. The medical or injection-based part may calm irritated tissue, while chiropractic care improves movement and body mechanics. Rehabilitation then helps restore strength, balance, and function (Jimenez, 2026a).

4. Rehabilitation Can Be Timed Around Pain Relief

Pain relief from an epidural injection may create an opportunity for active rehabilitation. During this period, the patient may be better able to perform controlled exercises, improve core stability, and practice safer movement patterns.

This timing is important. Restoring movement without rebuilding strength may leave the spine poorly supported. Strengthening without correcting painful joint restrictions may also be difficult. A coordinated plan allows each stage to support the next stage.

5. Progress Can Be Measured More Clearly

A multidisciplinary team can track whether the patient is improving in more than one area. Progress may be measured through:

  • Pain levels
  • Range of motion
  • Muscle strength
  • Neurological findings
  • Walking tolerance
  • Sitting tolerance
  • Sleep quality
  • Ability to work
  • Ability to complete daily activities
  • Reduced need for medication

If the injection decreases pain but weakness or numbness continues, the patient may need further medical testing. If mobility improves but radiating pain returns, the team can review the diagnosis and adjust the treatment plan.

Medical Oversight and Patient Safety

Epidural spinal injections are not right for every person. Before recommending an injection, the medical team should review the diagnosis, imaging, medications, allergies, blood sugar control, bleeding risk, and signs of infection.

Extra caution may be needed for patients with:

  • Blood-thinning medications
  • Bleeding disorders
  • Active infections
  • Poorly controlled diabetes
  • Medication allergies
  • Immune system problems
  • Pregnancy
  • Significant heart or kidney disease
  • Progressive neurological symptoms

The injection should be performed by a properly trained medical professional using appropriate imaging guidance and safety procedures. Patients should understand that corticosteroid injections can have side effects and that rare but serious complications are possible.

Care should also be coordinated after the procedure. The chiropractor, rehabilitation team, and medical provider should know when the injection was performed, which spinal level was treated, and whether the patient has any post-procedure restrictions.

Multidisciplinary Injury Care in El Paso

At Injury Medical Clinic PA in El Paso, Texas, Dr. Alexander Jimenez, DC, APRN, FNP-BC, CCST, CFMP, IFMCP, ATN, works within a multidisciplinary model that brings together chiropractic care, functional medicine, personal injury evaluation, rehabilitation, and medical collaboration.

Dr. Jimenez’s role includes evaluating neuromusculoskeletal injuries, examining joint movement, identifying muscle imbalances, reviewing neurological findings, and developing function-based rehabilitation plans. His published clinical observations emphasize that reducing pain is only one part of recovery. Joint mobility, strength, posture, stability, and safe movement must also be considered (Jimenez, 2026a, 2026b).

Dr. Maria Guadalupe Cardenas, MD, is Board Certified in Internal Medicine and has more than 40 years of medical experience. She serves as Medical Director and Collaborative Physician at Injury Medical Clinic PA. Public provider records identify her NPI as 1164426748, and her Texas medical license is listed as J2933. Her internal medicine background adds medical direction, medication review, risk assessment, chronic disease management, and referral coordination to the clinic’s injury-care model (Jimenez, n.d.; ChiroMed, 2026).

This type of team structure is used in many integrative and injury-care settings. The chiropractor manages appropriate mechanical and functional care, while the physician provides medical oversight. The team may also coordinate with pain-management specialists, radiologists, physical therapists, surgeons, attorneys, or other professionals when needed.

A Step-by-Step Treatment Plan

A coordinated recovery plan may follow these stages:

  1. Complete evaluation: Review the accident, symptoms, health history, neurological findings, movement, strength, and imaging.
  2. Rule out serious injuries: Check for fractures, instability, spinal cord problems, or other conditions that require urgent medical attention.
  3. Control acute pain: Use safe activity changes, medication, an epidural injection, or other medical treatment when appropriate.
  4. Restore movement: Begin gentle chiropractic care, mobility work, and soft-tissue treatment based on the diagnosis.
  5. Rebuild strength: Add exercises for the core, hips, back, neck, shoulders, and other affected areas.
  6. Correct compensation patterns: Improve posture, lifting methods, walking patterns, and work-related movements.
  7. Measure progress: Repeat physical and neurological examinations and adjust the plan when needed.
  8. Return to normal activity: Gradually increase work, exercise, driving, and daily activities.

Final Thoughts

Combining an epidural spinal injection with integrative chiropractic care may benefit selected patients with back injuries after a motor vehicle accident. The epidural injection may reduce short-term inflammation around an irritated spinal nerve. Chiropractic care may improve joint movement, reduce muscle tension, and address harmful compensation patterns. Rehabilitation can rebuild the strength and control needed for a safer return to normal activity.

The treatments should not be viewed as interchangeable or as guaranteed cures. Epidural injections work best for carefully selected nerve-related symptoms, while chiropractic care must be adapted to the patient’s diagnosis and stage of healing. A coordinated team can help determine when each treatment is appropriate and whether additional imaging, medical care, or specialist referral is needed.

At Injury Medical Clinic PA, the collaboration between Dr. Alex Jimenez and Dr. Maria Guadalupe Cardenas supports a whole-person approach that combines medical oversight, integrative chiropractic care, functional medicine, personal injury services, and rehabilitation. The main goal is not only to reduce pain, but also to help the patient regain movement, strength, stability, and confidence after an accident.
