Damaged in heavy car accident vehicles after collision on city street crash site at night. Road safety and insurance concept.
A woman in her mid-50s was the belted driver of a car struck head-on by a heavier SUV. The airbag deployed, and her car was a total loss. She arrived at our clinic eleven days later in constant, severe pain from her neck to her feet. This case shows how we triaged her injuries, screened for a vascular complication, delivered phased guideline-based care, and documented her recovery and remaining limits.
Table of Contents
The patient described pain that began at the scene: neck, upper and lower back, both hips, the abdomen, the right arm, and both legs down to the feet. Her right leg swelled and turned purple within hours, she could not move her right foot for about a week, and she had daily headaches and insomnia. Emergency services transported her from the scene, and she was seen by her primary care physician before coming to us.
On examination, she had marked tenderness and spasm along the whole spine, severely reduced neck and back motion in every direction, bruising beneath the skin of the back, and swelling of the right leg and both ankles. Walking was limited on stairs, curbs and uneven ground.
Her plan followed current practice guidelines for collision-related neck and back pain, which recommend manual therapy combined with advice, self-management and exercise.1,2,3
Weeks 0–2 · daily visits
Spinal and joint manipulation at reduced force, cold packs, electrical stimulation and gentle traction to calm pain, spasm and swelling; leg elevation and ice at home; close monitoring of the swollen leg.
Weeks 2–10 · weekly visits
Manual therapy and massage for persistent muscle tightness, low-force lumbar decompression for leg-referred pain, and a formal re-examination that confirmed progress and reduced visit frequency.
Weeks 13–21 · weekly visits
A printed stretching program with standing, seated and lying versions, hydration and electrolyte advice for new cramping, and release to as-needed care.
Her low back pain fell from 8 out of 10 in the first week to 1 out of 10 by about week 14, and was 2 out of 10 at the last rating. Neck pain, headaches, abdominal pain and left-leg pain resolved. Recovery was not a straight line: flares followed long periods of standing, work and stress, and each settled with care and home measures.
At release she was managing without pain medication, but low back pain, foot pain (worse on the left), hip and right-leg tightness, reduced standing and work endurance, and anxiety about driving remained. Prognostic research shows that high initial pain, widespread symptoms and post-injury low mood predict slower and less complete recovery,4 which matched her course.
