Battlefield Acupuncture is not traditional acupuncture; it is a specific, protocol-driven auricular therapy designed for immediate and potent pain modulation. The name itself hints at its origins, developed by Dr. Richard Niemtzow, a retired Air Force physician, to provide rapid pain relief to soldiers in the field where access to conventional pain medication might be limited. Today, its application has expanded far beyond the military, now used in emergency rooms, post-operative care units, and integrative pain management clinics like mine.
The technique involves inserting very small, semi-permanent needles, called ASP needles (Aiguille Semi-Permanente), into up to five specific points on the ear. These needles are tiny, dart-like studs that, once inserted, can remain in place for several days, providing continuous stimulation.
The efficacy of BFA lies in the rich and complex neurology of the external ear, or auricle. The ear is one of the few places on the body where multiple cranial nerves converge, including:
Stimulating these nerves sends direct signals to the brainstem and higher brain centers. The ear acts as a microsystem—a miniature map of the entire body. Specific points on the ear correspond to different organs, limbs, and physiological functions. When a BFA needle is inserted, it doesn’t just treat a local area; it sends a powerful signal to the central nervous system to recalibrate its pain response. This process is believed to trigger the release of the body’s own natural painkillers, known as endogenous opioids (like endorphins and enkephalins), and modulate the activity of brain regions involved in pain perception, such as the thalamus, cingulate gyrus, and periaqueductal gray.
The BFA protocol is systematic. We identify and needle five specific points in a particular sequence, constantly reassessing the patient’s pain level. While the full protocol uses five points, clinical experience shows that significant relief can often be achieved with just one or two needles.

When a patient comes to me for pain relief, the BFA procedure is methodical and patient-interactive. Here is a look at my clinical process:
First, I ask the patient to rate their pain on a scale of 0 to 10. We also identify the pain’s location and quality (e.g., sharp, dull, burning). This baseline is crucial for tracking progress.
The protocol begins with choosing which ear to treat first. If the pain is localized to one side of the body (lateralized), we start on the contralateral (opposite) ear. If the pain is central or on both sides, I typically start on the ear of the non-dominant hand.
This is the most critical part of the BFA protocol. After inserting the first needle (usually at the Cingulate Gyrus point), I ask the patient to stand up and walk around for a minute. Movement helps “activate” the needle’s effect and allows the brain to process the new neurological input. I then ask them to re-rate their pain.
This “treat, ambulate, reassess” cycle continues until the patient experiences satisfactory pain relief or all indicated points have been treated. The ASP needles typically remain in the ear for three to five days, providing continuous stimulation before they naturally fall out.
In my clinical experience, I’ve treated patients for various pain conditions using BFA. For example, one of my long-term patients suffers from chronic, widespread pain. She responds remarkably well to bilateral needle insertions at the Cingulate Gyrus and Thalamus points. We perform this treatment every two to three weeks, and it provides her with consistent and meaningful pain relief, allowing her to function more fully in her daily life.

For example, a patient with chronic joint pain might have an undiagnosed sensitivity to gluten, which is driving systemic inflammation. Or, their pain might be worsened by a deficiency in magnesium or Vitamin D. By addressing these root causes through targeted nutritional protocols, supplements, and lifestyle changes, we can reduce the body’s overall inflammatory burden. This makes both BFA and chiropractic care more effective and helps create lasting relief, rather than just temporarily managing symptoms.
