Explore how orthobiologic treatments can improve musculoskeletal health and enhance recovery with innovative therapies.
Table of Contents
Welcome to this in-depth exploration of a new model for patient care—the Precision Practice. In this educational post, I will guide you through a paradigm shift away from the traditional, high-volume, insurance-driven medical model toward a more sustainable, effective, and rewarding system focused on orthomolecular and orthobiologic medicine. Through my journey and clinical observations, I’ve developed a deep understanding of regenerative medicine, and I will guide you through the latest findings and clinical insights in the field. We will explore why many practices struggle: not because of the science, but because of flawed execution. I will detail a systematic framework for success, known as the Joint Vitality System, which emphasizes precision diagnosis, biologic matching, and guided recovery. We will delve into the physiological mechanisms underlying treatments such as Platelet-Rich Plasma (PRP) and cellular therapies, discussing how to match the right biologic to the right tissue need. This post also calls on clinicians to rethink their practice identity and deliver the care our patients deserve. I’ll also share our collaborative approach at Injury Medical Clinic, where my expertise in chiropractic and functional medicine is complemented by the medical oversight of Dr. Maria Guadalupe Cardenas, MD, ensuring a safe, effective, and integrated path to wellness for our patients.
My name is Dr. Alex Jimenez, and I hold several certifications, including Doctor of Chiropractic (DC), Advanced Practice Registered Nurse (APRN), Family Nurse Practitioner (FNP-BC), Certified Functional Medicine Practitioner (CFMP), and an Institute for Functional Medicine Certified Practitioner (IFMCP), among others. Throughout my years in practice at Injury Medical Clinic PA (also known as Mission Plaza Injury Medical Clinic) in El Paso, Texas, my perspective on what it means to be a healthcare provider has profoundly evolved.
Our practice is built on a unique, multidisciplinary, integrative model. A cornerstone of our practice is the collaboration with Dr. Maria Guadalupe Cardenas, MD, our esteemed Medical Director and Collaborative Physician. Dr. Cardenas is Board Certified in Internal Medicine (Texas MD License #J2933, NPI #1164426749) and has over 40 years of distinguished experience. Her extensive medical background provides essential oversight, helping our clinic function as a cohesive, integrative unit and ensuring our services meet the highest standards of safety and efficacy. This partnership between a DC/APRN and an MD is a powerful example of how multidisciplinary clinics can offer the best of both worlds. Together, we integrate chiropractic adjustments, functional medicine protocols, rehabilitative exercises, and advanced medical treatments to address the root cause of our patients’ conditions, especially in the context of personal injury and complex musculoskeletal conditions.
Patients don’t wake up one morning hoping for a PRP (Platelet-Rich Plasma) injection or any other specific medical procedure. No one ever thought, “You know what would be great today? A rotator cuff tear, so I can’t play pickleball or lift my grandkids.” People don’t seek out pain or limitations. They seek recovery, function, and the ability to return to the life they love.
This is where our integrative model shines. We can say:
This approach isn’t just helpful; it’s massively valuable. We are not just identifying a problem; we are providing the solution and the support system to achieve it.
Throughout my years in practice and my continuous study of regenerative medicine, I’ve observed a recurring theme. When orthobiologic treatments fail to deliver on their promise, it is rarely an issue with the underlying science. The failure is almost always in the execution.
Many of us in this field believe that orthobiologics should be a first-line treatment for a vast array of musculoskeletal (MSK) conditions. The primary hurdle, however, is that these services are typically cash-based. This is where poor execution can derail an otherwise effective therapy.
Common Pitfalls in Execution:
I believe we are at an inflection point. If we commit to doing the work—collecting data, using ultrasound guidance, and meticulously selecting patients—orthobiologics will become a standard first-line treatment within the next five years.
To address these execution challenges, I developed a framework I call the Joint Vitality System. It’s based on the philosophy that regenerative medicine is not just a procedure; it is a comprehensive system of care.
The core components of the Joint Vitality System are:
I often call myself a “show-me state” practitioner. I don’t believe it until I can prove it. This means using every tool at my disposal to understand not only what is wrong but why it failed.
Once we have a precise diagnosis, we must match the biologic to the tissue’s specific needs.
Our treatment philosophy is a packaged approach, a partnership where the patient and our clinical team work together toward a common goal.
The journey to recovery begins weeks before the procedure in the health optimization phase. We “stack the deck” in the patient’s favor by conducting comprehensive blood work and creating a personalized plan to address any deficiencies and reduce systemic inflammation. A healthy, well-nourished, and hormonally balanced body will respond far more effectively to regenerative treatments.
The procedure itself—whether PRP or another orthobiologic treatment—is a pivotal moment. But it’s Day One, not the final step. This intervention acts as a powerful catalyst, signaling the body to initiate a robust healing cascade.
The post-procedure period is where the real work of transformation continues. Our job is not done once the injection is administered. This guided recovery phase includes personalized rehabilitation, lifestyle coaching, and regular follow-ups. This continuous support system is what turns a temporary improvement into a lasting transformation.
As a Doctor of Chiropractic, my primary focus is on the body’s structure and its relationship to nervous system function and overall health. In our integrative model, chiropractic care is a vital component woven throughout the recovery process.
Our integrated clinic model, with Dr. Cardenas providing medical direction, enables us to coordinate these aspects of care, ensuring the rehabilitation plan is medically sound and complements the regenerative procedure.
In the conventional insurance-based model, profitability is tied to volume. I’ve found that the ratio can be staggering—I would have to see approximately fifteen insurance-based patients to generate the same revenue as a single orthobiologics patient. This is where the orthomolecular micro-practice, or what I call the Precision Practice, comes in.
These figures represent a sustainable business that allows you to practice medicine on your own terms. For example, in my practice, we dedicate an entire day to just four procedure patients. Can you imagine the level of attention and care those individuals receive?
Many of us in the medical field believe that business is more difficult or intimidating than medicine. I want to assure you it is not. You don’t need an MBA to start a small, specialized medical practice. The risks in business are primarily financial, not physical. You don’t need to have every single step figured out before you start. You need a few key elements: a sense of humor, a way to handle phone calls, a system for record-keeping, and the necessary clinical tools. For an orthobiologics practice, the most significant investment is often an ultrasound machine, which is crucial for diagnostic accuracy and procedural guidance.
The most durable and effective way to grow an orthobiologics practice is through relationships.
In the rapidly advancing field of orthobiologics, a significant challenge is data loss. Many great clinicians are not systematically collecting data on their outcomes. As a solo practitioner, implementing a data collection system was a deliberate choice—and an investment.
Why is data so important?
We use a system to track key metrics, including Pain and Function Scores and Patient-Reported Outcomes (PROs). We are transparent about the cost, adding a nominal fee of about $25 to the treatment package. Virtually every patient agrees. They want their doctor to be collecting data.
Adopting the Precision Practice model requires an identity shift. It means broadening our perspective to encompass the full spectrum of a person’s health—their biochemistry, biomechanics, lifestyle, and goals. We demonstrated these principles with a live patient—my 80-year-old father, an avid pickleball player who injured his shoulder. We developed a comprehensive plan to get him ready for a two-week bike trip in Amsterdam. This is what it’s all about: helping our patients get back to doing the things they love.
The future of orthopedics lies in this integrated, systematic approach. It starts with seeing your practice as it can be and what your patients truly deserve: transformative care.
