What Is MFAT—and Why Is It Different From PRP?

Abstract

Microfragmented adipose tissue, or MFAT, is an autologous orthobiologic procedure using a small amount of the patient’s own fat tissue. This guide covers harvesting, mechanical processing, placement, recovery, rehabilitation, patient selection, and a comparison with platelet-rich plasma (PRP), without assuming either is universally better.

A knee can handle thousands of steps during a warehouse shift, ladder work in a data center, or hours between network racks—until joint pain starts limiting stairs, squatting, walking, training, or sleep. That is when patients often begin researching MFAT and PRP.

Both use material from the patient, but they differ in source, preparation, and the burden of the procedure. Neither should be promised as a guaranteed way to rebuild damaged cartilage.

What MFAT Actually Is

MFAT stands for microfragmented adipose tissue. Adipose is body fat, but it also contains connective tissue, blood-vessel-associated cells, signaling molecules, and other cellular components. During MFAT preparation, a clinician removes a small amount of subcutaneous fat, typically from the abdomen or flank, then mechanically washes and reduces it to smaller fragments suitable for injection (McGee et al., 2022).

This is not cosmetic liposuction. Published techniques use local anesthesia, tumescent fluid, a small opening, and cannula aspiration to collect limited tissue for a musculoskeletal procedure (McGee et al., 2022).

The MFAT Procedure: A Patient-Friendly Walk-Through

1. Evaluation Comes Before the Needle

Selection starts with history, examination, and appropriate imaging. Clinicians must determine whether symptoms are mainly related to osteoarthritis, tendon disease, referred pain, instability, a major tear, fracture, inflammatory arthritis, infection, or another condition.

Weight-bearing X-rays show joint-space loss and bony change. Ultrasound can evaluate soft tissues and guide injections. MRI may help when cartilage, meniscal, labral, or bone pathology could change the plan.

Severity matters. An injection cannot mechanically reopen a severely collapsed joint, correct major deformity, or replace a missing ligament. Advanced structural disease deserves realistic goals and, when appropriate, a surgical opinion.

2. The Harvest Area Is Numbed

The surgeon cleans the harvest site and places local anesthetic in the skin and fatty layer. Tumescent solution may improve comfort and reduce bleeding. A small opening allows a slender cannula to enter the subcutaneous fat. Patients may still notice pressure or pulling, and protocols vary.

3. A Small Amount of Fat Is Collected

The cannula moves through the fatty layer to collect lipoaspirate, often from the abdomen or flank. This creates a second treatment site that PRP does not require, an important practical difference when comparing the procedures (Baria et al., 2024).

4. The Tissue Is Mechanically Processed

The harvested fat is modified before reinjection. MFAT systems mechanically wash, rinse, filter, resize, or microfragment it, reducing blood, oil, fluid, and larger tissue pieces. The goal is an injectable tissue product created through mechanical point-of-care processing rather than laboratory cell culture (McGee et al., 2022).

5. The Target Is Treated

Clinicians can then place processed tissue into an appropriately selected joint or soft-tissue region. Ultrasound guidance can help confirm accurate intra-articular placement. The target should match the diagnosis; vague pain near a joint is not enough.

How Is MFAT Different From PRP?

PRP starts with blood. A blood draw is centrifuged to concentrate platelets in plasma, producing an injectable preparation containing platelet-derived signaling factors. MFAT starts with adipose tissue and requires a minor harvest procedure.

Key practical differences include:

  • PRP usually requires a blood draw, processing, and injection.
  • MFAT requires local anesthesia, fat harvest, mechanical processing, and injection.
  • MFAT adds a harvest site that may bruise or feel sore.
  • Both procedures can cause temporary post-injection soreness or swelling.

Current evidence does not establish one as universally superior. A randomized trial found that MFAT and PRP groups both improved at 12 months, with no significant difference between treatments (Baria et al., 2024). A 2025 meta-analysis similarly reported broadly comparable pain, function, and safety outcomes across most follow-up periods, while emphasizing that more research is needed (Park et al., 2025).

The best option depends on diagnosis, severity, prior care, tolerance for the procedure, goals, cost, clinician judgment, and condition-specific evidence.

What Recovery May Feel Like

The treated joint can feel achy or temporarily irritated, while the harvest area may feel bruised or sore. Studies show these harvest-site symptoms are generally temporary, and major complications are uncommon (Baria et al., 2024). Judge improvement by function over time, not soreness during the first few days after treatment.

Patients should receive instructions about wound care, medications, activity, and warning signs. Fever, drainage, increasing redness, severe swelling, escalating pain, chest pain, shortness of breath, or new neurological symptoms require prompt medical attention.

Rehabilitation remains important. Progressive loading, mobility work, strength training, gait mechanics, sleep, nutrition, and weight management can help translate symptom improvement into better capacity for walking, stairs, kneeling, lifting, carrying, and work.

Who May—or May Not—Be a Candidate?

MFAT is not a shortcut around diagnosis. Selection should consider:

  • The pain generator and imaging findings
  • Osteoarthritis grade and remaining joint structure
  • Mechanical alignment or instability
  • Previous rehabilitation and injections
  • Infection risk, bleeding risk, and medications
  • Metabolic health and healing capacity
  • Work demands and functional goals
  • Whether another treatment is more appropriate

Advanced degeneration does not automatically answer the question either way. However, expectations should be cautious, and an orthobiologic procedure should not delay a needed surgical consultation when severe structural disease, progressive deformity, or major functional loss makes that discussion appropriate.

A Necessary Regulatory Conversation

Patients should ask what product or device is being used and how the proposed use is regulated. The U.S. Food and Drug Administration warns that regenerative medicine products have not been approved for orthopedic conditions such as osteoarthritis and advises consumers to be cautious about unproven claims (U.S. Food and Drug Administration, 2021).

Patients deserve evidence, risks, alternatives, and choice.

Integrated Care Means More Than an Injection

At Injury Medical Clinic PA in El Paso, Dr. Alex Jimenez, DC, APRN, FNP-BC, CFMP, IFMCP, combines chiropractic, advanced-practice nursing, diagnostics, rehabilitation, functional nutrition, and selected orthobiologic procedures. Dr. Maria Guadalupe Cardenas, MD, board-certified in internal medicine, provides collaborative medical oversight for complex cases.

An integrated model should not push every patient toward MFAT, PRP, chiropractic care, acupuncture, or another procedure. Beneficence means matching treatment to diagnosis and goals. Non-maleficence means avoiding unnecessary invasiveness or medication burden when safer options may help. Autonomy means giving patients enough information to decide while coordinating with their medical team.

The Bottom Line

MFAT uses the patient’s own adipose tissue; PRP uses the patient’s own blood. MFAT requires a small fat harvest and mechanical processing, while PRP usually requires a blood draw and centrifugation. Both have evidence for selected musculoskeletal conditions, especially knee osteoarthritis, but neither is a universal solution.

Instead of asking, “Which injection is stronger?” ask, “What is causing my pain, how severe is the structural problem, what are my realistic options, and what rehabilitation plan gives me the best chance to function better?”

Schedule a coordinated medical, chiropractic, imaging, and rehabilitation evaluation to answer those questions before choosing any procedure.
