When patients come to me with radiating pain, numbness, tingling, or weakness, they often describe it as a “pinched nerve.” While accurate, this term simplifies a complex physiological process. A nerve entrapment occurs when a peripheral nerve is compressed or restricted by surrounding anatomical structures, such as muscles, ligaments, tendons, or bones.

This is precisely the scenario in many cases of persistent lateral elbow pain that doesn’t respond to traditional treatments for “tennis elbow.” For these patients, the pain may not originate from the tendon (lateral epicondyle). Instead, it often comes from the radial nerve entrapped as it passes through the forearm.
To understand the specific case of lateral elbow pain discussed by leading researchers, we need to examine the anatomy. The radial nerve travels down the arm and, near the elbow, it passes through a space called the radial tunnel. A key landmark within this tunnel is the Arcade of Frohse, a fibrous arch at the entrance of the supinator muscle.
The deep branch of the radial nerve (also called the posterior interosseous nerve) must pass through this tight archway. With repetitive forearm movements, such as pronation (turning the palm down) and supination (turning the palm up), the supinator muscle can compress the nerve against this rigid arch. This leads to Radial Tunnel Syndrome. It is characterized by deep, burning, or aching pain in the top of the forearm, distinct from the sharp pain at the bony epicondyle seen in classic tennis elbow.
