I am Dr. Alex Jimenez, and my background spans chiropractic (DC), advanced practice nursing as a Family Nurse Practitioner (FNP-BC), and certifications in Functional Medicine (CFMP, IFMCP). Today, I want to take you on a journey into one of our advanced treatment sessions. We are working with a patient presenting with significant shoulder pain and dysfunction. Our first step is always a thorough diagnostic evaluation, which in this case involves musculoskeletal ultrasound. This modern, evidence-based tool allows us to visualize the soft tissues—muscles, tendons, and ligaments—in real-time, providing a dynamic view of the pathology.
As I guide the ultrasound probe over the patient’s shoulder, a clear picture of the underlying issues emerges.
This detailed, real-time imaging allows me to map out the precise locations for our regenerative injections. It transforms the procedure from a “blind” injection into a highly targeted, ultrasound-guided intervention.
Based on our ultrasound findings, we have a clear plan. We will address multiple pathological sites to provide comprehensive healing. The primary goal is not just to mask the pain but to stimulate true tissue repair using the patient’s own biological materials.
Before we begin the regenerative portion of the treatment, patient comfort is paramount. To achieve this, I perform a suprascapular nerve block. The suprascapular nerve provides a significant portion of the sensory innervation to the shoulder joint. By temporarily anesthetizing this nerve, we can make the entire procedure nearly painless for the patient.
Using the ultrasound, I can pinpoint the exact location of the neurovascular bundle in the suprascapular notch. The nerve lies just medial to the artery. I carefully guide a small-gauge needle to this spot and inject a local anesthetic. This technique is highly effective and allows the patient to remain relaxed and comfortable, which is crucial for the precision required in the subsequent steps. This block is my standard of care; it dramatically improves the patient experience and allows me to work meticulously without causing distress.
Now, we turn to the core of the treatment: the regenerative injectate. For this patient, we are using a combination of Platelet-Rich Plasma (PRP) and other cellular components derived from his blood and tissue.
My strategy for performing multiple injections is to work from back to front. I start with the areas that are generally less sensitive and move toward the more sensitive structures at the front of the shoulder. This approach helps maintain patient comfort throughout the process.
I begin with the intra-articular injection into the main shoulder joint (the glenohumeral joint). The goal here is to reduce inflammation, provide lubrication, and create a healing environment within the joint capsule. Using a posterior approach, I guide the needle under ultrasound visualization, slipping past the labrum and into the joint space.
Here, I inject a portion of the cellular concentrate. The rationale is to combat the general inflammatory state of the joint, which is often present with chronic rotator cuff pathology. These cells release anti-inflammatory cytokines and growth factors that soothe the irritated synovial lining and provide the foundational support for the more targeted tendon treatments to follow.
Next, I address the primary issue: the partial tear in the supraspinatus tendon. I reposition the ultrasound probe to get a clear view of the “footprint” where the tendon attaches to the bone. With millimeter precision, I guide the needle directly into the gap within the tendon.
I perform a technique known as hydrodissection, injecting small aliquots of PRP. You can see the fluid spreading through the damaged tissue on the ultrasound screen, breaking up scar tissue and delivering growth factors directly to the site of injury. The goal is to “pepper” the entire damaged area with PRP to stimulate a robust healing response. The growth factors will signal fibroblasts—the cells responsible for creating collagen—to come to the area and lay down new, healthy tendon fibers.
Now, we move to the top of the shoulder to treat the arthritic AC joint. This is a small, tight joint, and precision is critical. Using an “out-of-plane” ultrasound technique, I direct the needle vertically down into the joint space, which appears as a bright white dot on the screen. I inject a small amount of PRP to reduce inflammation and potentially stimulate cartilage repair. This addresses the sharp, localized pain the patient experiences.
Finally, I address the front of the shoulder. The patient is positioned with their arm externally rotated to expose the subscapularis tendon. I perform a cross-sectional view with the ultrasound and carefully inject PRP into the tendinopathic areas. Because the subscapularis is a large, multi-pennate muscle, it’s important to treat it along its breadth to restore its function and stability.
This completes the comprehensive treatment. We have successfully targeted the intra-articular space, the primary tendon tear, the arthritic AC joint, and the secondary tendinopathies—all in one session.
The regenerative injection is a powerful catalyst for healing, but it is not the endpoint. The subsequent rehabilitation phase is where integrative chiropractic care becomes absolutely essential.
The initial injury often develops due to faulty biomechanics—poor posture, muscle imbalances, or improper movement patterns. If we only treat the damaged tissue without correcting the underlying mechanical cause, the injury is likely to recur.

Following the injections, the patient is instructed to expect some soreness for 48 to 72 hours. This is a positive sign—it indicates that the inflammatory healing cascade has been successfully initiated. We advise them to avoid anti-inflammatory medications (such as ibuprofen), as they can blunt the very response we are trying to stimulate.
The journey to full recovery is a process. Tissue remodeling takes time. Patients typically begin to notice significant improvements in pain and function within 4-6 weeks, with continued progress over several months as the new tissue matures. We follow up closely with our patients, adjusting their rehabilitation plans as they heal and regain strength.
