Restoring Joint Function with Regenerative Medicine Therapies

Restoring Joint Function with Regenerative Medicine

Abstract

This post will guide you through the intricate world of shoulder pathology and the innovative, evidence-based treatments we employ at Injury Medical Clinic. From a first-person perspective, I will walk you through a detailed clinical procedure, demonstrating how we utilize advanced diagnostic imaging and regenerative medicine techniques to address complex shoulder injuries. We will explore the physiological basis of conditions such as rotator cuff tears, tendinopathies, and arthritis, and I will explain the rationale for our multifaceted treatment protocols. This journey will highlight the power of an integrative care model that combines the expertise of chiropractic, functional medicine, and internal medicine under the medical direction of Dr. Maria Guadalupe Cardenas, MD. Our goal is to provide a comprehensive roadmap for shoulder restoration, showcasing how targeted, minimally invasive therapies can stimulate the body’s natural healing mechanisms to support long-term recovery.

Our Multidisciplinary Team: A Foundation for Comprehensive Care

At Injury Medical Clinic, our philosophy is rooted in collaboration. I am Dr. Alex Jimenez, and my background spans chiropractic (DC), advanced practice nursing as a Family Nurse Practitioner (FNP-BC), and certifications in Functional Medicine (CFMP, IFMCP). This unique combination allows me to view patient health through multiple lenses—structural, functional, and systemic.

However, the strength of our practice lies in our team approach. We are privileged to have Dr. Maria Guadalupe Cardenas, MD, as our Medical Director and Collaborative Physician. With over 40 years of experience as a board-certified internist, Dr. Cardenas provides invaluable medical oversight, ensuring our protocols meet the highest standards of safety and efficacy. Her extensive experience in internal medicine is crucial, especially when managing patients with complex comorbidities that could affect their recovery. This multidisciplinary setup is common in forward-thinking integrative and injury-care clinics, creating a powerful synergy among chiropractic adjustments, medical diagnostics, rehabilitation, and advanced regenerative therapies.

Together, our team integrates:

  • Chiropractic Care: Focused on restoring spinal and extremity joint biomechanics, which is often the foundational step to correcting movement patterns that lead to injury.
  • Medical Oversight (Dr. Cardenas): Ensuring all procedures are medically sound, managing systemic health factors, and providing a comprehensive diagnostic framework.
  • Functional Medicine: Investigating the root causes of inflammation and dysfunction, such as nutritional deficiencies or metabolic imbalances.
  • Personal Injury & Rehabilitation: Creating customized programs to restore strength, mobility, and function after an injury.
  • Regenerative Medicine: Utilizing the body’s own healing components to repair damaged tissues at a cellular level.

This collaborative environment allows us to create a truly personalized and effective treatment plan for every patient who walks through our doors.

A Clinical Walkthrough: Diagnosing and Treating a Complex Shoulder

Today, I want to take you on a journey into one of our advanced treatment sessions. We are working with a patient presenting with significant shoulder pain and dysfunction. Our first step is always a thorough diagnostic evaluation, which in this case involves musculoskeletal ultrasound. This modern, evidence-based tool allows us to visualize the soft tissues—muscles, tendons, and ligaments—in real-time, providing a dynamic view of the pathology.

Visualizing the Injury: The Power of Ultrasound

As I guide the ultrasound probe over the patient’s shoulder, a clear picture of the underlying issues emerges.

  • The Supraspinatus Footprint: We start by examining the supraspinatus tendon, a key component of the rotator cuff. On the screen, we can see the humeral head—the “ball” of the shoulder joint—as a bright white, curved line. The tendon should appear as a uniform, fibrillar structure inserting onto this bone, an area we call the “footprint”. In this patient, I can identify a distinct gap or partial tear within the tendon fibers. This disruption is a primary source of his pain and weakness. Each of those little “knots” or disruptions you see could represent a damaged protein domain within the tendon’s collagen matrix.
  • The Subscapularis and Biceps Tendons: Moving to the front of the shoulder, we assess the subscapularis and long head of the biceps tendons. The subscapularis is crucial for internal rotation and shoulder stability. We observe some tendinopathic changes here as well—a thickening and disorganization of the tendon fibers.
  • The Acromioclavicular (AC) Joint: At the top of the shoulder, the AC joint shows signs of arthritis. The ultrasound reveals osteophytes (bone spurs) and a narrowing of the joint space, which can cause sharp, localized pain, especially with overhead movements.

This detailed, real-time imaging allows me to map out the precise locations for our regenerative injections. It transforms the procedure from a “blind” injection into a highly targeted, ultrasound-guided intervention.

Designing the Treatment: A Multi-Target Regenerative Protocol

Based on our ultrasound findings, we have a clear plan. We will address multiple pathological sites to provide comprehensive healing. The primary goal is not just to mask the pain but to stimulate true tissue repair using the patient’s own biological materials.

The Role of a Suprascapular Nerve Block

Before we begin the regenerative portion of the treatment, patient comfort is paramount. To achieve this, I perform a suprascapular nerve block. The suprascapular nerve provides a significant portion of the sensory innervation to the shoulder joint. By temporarily anesthetizing this nerve, we can make the entire procedure nearly painless for the patient.

Using the ultrasound, I can pinpoint the exact location of the neurovascular bundle in the suprascapular notch. The nerve lies just medial to the artery. I carefully guide a small-gauge needle to this spot and inject a local anesthetic. This technique is highly effective and allows the patient to remain relaxed and comfortable, which is crucial for the precision required in the subsequent steps. This block is my standard of care; it dramatically improves the patient experience and allows me to work meticulously without causing distress.

Preparing the Regenerative Injectate: PRP and Cellular Components

Now, we turn to the core of the treatment: the regenerative injectate. For this patient, we are using a combination of Platelet-Rich Plasma (PRP) and other cellular components derived from his blood and tissue.

  1. Blood Draw and Processing: Earlier, my team performed a simple blood draw. That blood is then processed in a centrifuge to separate its components. The goal is to concentrate the platelets into a small volume of plasma. Platelets are the body’s first responders to injury; they are packed with powerful growth factors that orchestrate the healing cascade. These growth factors signal stem cells to the area, promote the formation of new blood vessels (angiogenesis), and stimulate the production of new collagen and tissue matrix.
  2. Color-Coding for Safety and Precision: In my practice, organization is key to safety and efficiency. I use a color-coding system for my syringes. For example, my orange-capped syringe might contain the PRP destined for intratendinous injection, while a gray-capped syringe holds the cellular components for the intra-articular space. This simple system eliminates any ambiguity and ensures the right biological product is delivered to the correct target. Today, our lab analysis shows we have a robust platelet concentration—1.8 billion platelets per milliliter —an excellent yield for stimulating a powerful healing response.

The Injection Sequence: A Back-to-Front Approach

My strategy for performing multiple injections is to work from back to front. I start with the areas that are generally less sensitive and move toward the more sensitive structures at the front of the shoulder. This approach helps maintain patient comfort throughout the process.

Step 1: Intra-Articular Injection for Glenohumeral Health

I begin with the intra-articular injection into the main shoulder joint (the glenohumeral joint). The goal here is to reduce inflammation, provide lubrication, and create a healing environment within the joint capsule. Using a posterior approach, I guide the needle under ultrasound visualization, slipping past the labrum and into the joint space.

Here, I inject a portion of the cellular concentrate. The rationale is to combat the general inflammatory state of the joint, which is often present with chronic rotator cuff pathology. These cells release anti-inflammatory cytokines and growth factors that soothe the irritated synovial lining and provide the foundational support for the more targeted tendon treatments to follow.

Step 2: Targeting the Supraspinatus Tear

Next, I address the primary issue: the partial tear in the supraspinatus tendon. I reposition the ultrasound probe to get a clear view of the “footprint” where the tendon attaches to the bone. With millimeter precision, I guide the needle directly into the gap within the tendon.

I perform a technique known as hydrodissection, injecting small aliquots of PRP. You can see the fluid spreading through the damaged tissue on the ultrasound screen, breaking up scar tissue and delivering growth factors directly to the site of injury. The goal is to “pepper” the entire damaged area with PRP to stimulate a robust healing response. The growth factors will signal fibroblasts—the cells responsible for creating collagen—to come to the area and lay down new, healthy tendon fibers.

Step 3: Addressing the Acromioclavicular (AC) Joint

Now, we move to the top of the shoulder to treat the arthritic AC joint. This is a small, tight joint, and precision is critical. Using an “out-of-plane” ultrasound technique, I direct the needle vertically down into the joint space, which appears as a bright white dot on the screen. I inject a small amount of PRP to reduce inflammation and potentially stimulate cartilage repair. This addresses the sharp, localized pain the patient experiences.

Step 4: Treating the Subscapularis and Biceps Tendons

Finally, I address the front of the shoulder. The patient is positioned with their arm externally rotated to expose the subscapularis tendon. I perform a cross-sectional view with the ultrasound and carefully inject PRP into the tendinopathic areas. Because the subscapularis is a large, multi-pennate muscle, it’s important to treat it along its breadth to restore its function and stability.

This completes the comprehensive treatment. We have successfully targeted the intra-articular space, the primary tendon tear, the arthritic AC joint, and the secondary tendinopathies—all in one session.

The Role of Integrative Chiropractic Care in Recovery

The regenerative injection is a powerful catalyst for healing, but it is not the endpoint. The subsequent rehabilitation phase is where integrative chiropractic care becomes absolutely essential.

The initial injury often develops due to faulty biomechanics—poor posture, muscle imbalances, or improper movement patterns. If we only treat the damaged tissue without correcting the underlying mechanical cause, the injury is likely to recur.

This is how chiropractic fits into the recovery protocol:

  • Restoring Joint Mobility: I will perform specific chiropractic adjustments to the thoracic and cervical spine and the clavicle. Stiffness in the thoracic spine, for instance, forces the shoulder to overcompensate, placing excessive strain on the rotator cuff. By restoring proper mobility in the kinetic chain, we offload the healing shoulder.
  • Correcting Scapular Dyskinesis: Many shoulder problems are linked to poor movement of the scapula (shoulder blade). We use manual therapy and targeted exercises to retrain the muscles that control the scapula, ensuring it glides smoothly and provides a stable base for arm movement.
  • Post-Injection Rehabilitation: The PRP injection initiates an inflammatory healing phase that lasts for several days. During this time, we recommend relative rest. However, as new tissue forms, it must be properly loaded to mature into strong, organized tendon fibers. Our rehabilitation team guides the patient through a progressive program of gentle range-of-motion exercises, followed by strengthening exercises. This controlled stress is crucial for aligning the new collagen fibers and building a resilient tendon. Without this, the new tissue may heal as disorganized scar tissue, which is weak and prone to re-injury.

Our clinic’s structure, with my dual credentials and Dr. Cardenas’s medical direction, allows us to seamlessly blend these modalities. The regenerative procedure jump-starts the biological repair, and the chiropractic and rehabilitative care ensures the mechanical and functional environment is optimized for that repair to be successful and lasting.

What to Expect After the Procedure

Following the injections, the patient is instructed to expect some soreness for 48 to 72 hours. This is a positive sign—it indicates that the inflammatory healing cascade has been successfully initiated. We advise them to avoid anti-inflammatory medications (such as ibuprofen), as they can blunt the very response we are trying to stimulate.

The journey to full recovery is a process. Tissue remodeling takes time. Patients typically begin to notice significant improvements in pain and function within 4-6 weeks, with continued progress over several months as the new tissue matures. We follow up closely with our patients, adjusting their rehabilitation plans as they heal and regain strength.

By combining cutting-edge regenerative medicine with the foundational principles of chiropractic and functional health, we offer our patients a truly holistic and powerful path to overcoming chronic shoulder pain and reclaiming their active lives.
