The Orthobiologic Continuum: PRP, MFAT, and Chiropractic Care for Overuse Injuries

Abstract

Repetitive strain builds when tissue loading exceeds recovery. Tech workers may develop wrist, forearm, or median nerve symptoms from keyboard and mouse use, while Amazon logistics staff may experience pain from repeated lifting, scanning, and reaching. This article explains how PRP, MFAT, chiropractic care, rehabilitation, and medical oversight can support healing while correcting mechanics that keep injury active.

Overuse injuries are usually more than just “inflamed tissue.” A tendon may be overloaded, a joint may move poorly, or a nerve may be irritated as provoking motions repeat through each shift. Treating biology without mechanics leaves stress unchanged; correcting mechanics alone may not resolve persistent tissue damage.

Why Repetition Becomes a Tissue Problem

Tendons transmit force but require recovery. Repeated loading can outrun remodeling, especially when sleep, conditioning, ergonomics, prior injury, or work demands reduce recovery. Tendon healing moves through inflammatory, proliferative, and remodeling phases, and repaired tissue may not regain its original mechanical properties (Darrieutort-Laffite et al., 2024).

For technology workers, prolonged wrist extension, mouse use, gripping tools, and sustained neck or shoulder tension can contribute. Complaints may include wrist or forearm aching, nighttime numbness, reduced grip endurance, or pain after computer-intensive days.

Warehouse and logistics workers face heavier cycles. Box handling, scanner use, carts, overhead reaching, and low-bin lifting can overload the wrist, elbow, shoulder, and spine. In both groups, tissue and movement patterns deserve evaluation.

PRP: Concentrated Signals, Not a Magic Repair

Platelet-rich plasma is prepared from the patient’s own blood. Concentrated platelets release signaling molecules linked to collagen synthesis and tissue remodeling. The rationale is strong, but outcomes vary with diagnosis, preparation, injection technique, tissue type, and rehabilitation.

A review of 33 randomized trials found no clear PRP superiority across many tendinopathy comparisons. Carpal tunnel studies did show better pain and symptom-severity measures at several follow-ups, although certainty was low or very low (Masiello et al., 2023).

A 2025 meta-analysis of seven randomized studies involving 365 patients also reported improved patient-reported carpal tunnel symptoms and function with PRP. Objective measures such as nerve conduction and median-nerve size changed less consistently (Du et al., 2025).

Symptom relief does not prove complete nerve recovery.

Where PRP May Fit

PRP may be considered for selected chronic tendon problems or mild-to-moderate carpal tunnel syndrome when conservative care has not produced adequate progress. It is not an automatic first step.

Before injection, the team should determine:

  • Pain source: tendon, joint, nerve, muscle, or referred pain
  • Signs of advanced nerve compromise
  • Work mechanics that keep reloading tissue
  • Prior bracing, rehabilitation, ergonomic changes, and relevant medical risks

These questions support safety and patient autonomy.

MFAT: A Different Orthobiologic Tool

Microfragmented adipose tissue is mechanically prepared from the patient’s own fat and preserves a matrix containing multiple cell types and signaling elements. It should not be described as a guaranteed “stem-cell cure.”

Evidence for MFAT is stronger for symptomatic knee osteoarthritis than for repetitive-strain wrist or elbow disorders. A 2025 review found moderate evidence for improved knee pain and function, while emphasizing fair study quality and limited external validity (Hohmann et al., 2025).

An evidence-based continuum means matching treatment to diagnosis and evidence, not choosing the most advanced procedure. A logistics worker with joint degeneration may require a different plan than a tech worker with suspected median nerve entrapment.

For carpal tunnel symptoms, the pathway may favor splinting, manual therapy, ergonomic correction, PRP in selected cases, or surgical referral when nerve compromise is advanced.

Chiropractic Care Corrects the Load Environment

Orthobiologics address tissue biology. Chiropractic and rehabilitative care address the mechanical environment in which the tissue must recover.

A systematic review of manual therapy for carpal tunnel syndrome reported improved symptoms and function, although techniques varied (Du et al., 2022). A structural assessment may examine cervical and thoracic mobility, shoulder-blade control, elbow and forearm mechanics, wrist position, grip, lifting technique, and trunk control.

For a programmer, the provoking chain may begin at the neck or shoulder and end at the wrist. For an Amazon associate, poor trunk mechanics can shift excessive demand into smaller tissues.

Chiropractic adjustments are therefore not an injection “booster.” They are one part of a mechanical correction strategy that can include mobilization, exercise, neural mobility, ergonomic retraining, progressive loading, and job-specific rehabilitation.

The Dual-Scope Advantage: One Plan, Two Clinical Lenses

Dr. Jimenez’s dual DC/APRN scope is useful because overuse injuries cross boundaries. A wrist complaint may require neurologic examination, imaging, medication review, ergonomic analysis, and rehabilitation planning. The chiropractic lens asks what movement pattern perpetuates load. The medical lens asks what diagnosis, systemic factor, procedure, or risk changes the plan.

This supports three ethical priorities:

  • Beneficence: coordinate diagnostics and treatment for better function and recovery.

  • Autonomy: explain evidence, uncertainty, alternatives, costs, and referral thresholds so patients can choose knowingly.

A Practical Orthobiologic Continuum for Workers

A staged plan has four levels.

1. Define the Pain Generator

History, examination, provocative testing, functional assessment, and imaging or electrodiagnostic studies, when indicated, identify what is actually injured.

2. Correct Daily Load

Workstation changes, lifting mechanics, task rotation, bracing, movement breaks, sleep, and recovery reduce repeated irritation.

3. Restore Capacity

Chiropractic care and progressive rehabilitation build mobility, strength, nerve excursion, coordination, and tolerance for real job demands.

4. Add Orthobiologics Selectively

PRP or MFAT may be considered when the diagnosis, tissue target, evidence, and patient goals support them. Rehabilitation continues because no injection can correct the repetitive force pattern on its own.

When Symptoms Need Faster Escalation

Seek prompt evaluation for progressive weakness, constant numbness, muscle wasting, lost coordination, traumatic rupture, fever, major swelling, or rapidly worsening pain. These findings may require imaging, electrodiagnostic testing, specialist referral, or surgery.

For workers who depend on their hands, shoulders, and backs, timely diagnosis can prevent a manageable overload problem from becoming prolonged disability.

Integrated Healing Means Treating Biology and Mechanics

The orthobiologic continuum does not position PRP, MFAT, and chiropractic care as competitors. It sequences tools around a verified diagnosis.

For tech workers and logistics staff, the plan begins by reducing avoidable load and restoring mechanics. Biologic procedures are added only when evidence supports their role.
