Abstract

Adhesive capsulitis, commonly known as frozen shoulder, is a debilitating condition characterized by severe pain and a progressive loss of shoulder motion. In this educational post, I will explore the physiological underpinnings of this condition and introduce a powerful, minimally invasive procedure called ultrasound-guided hydrodistension, also known as hydroplasty. We will review the latest findings from leading sports medicine experts and provide a detailed, step-by-step breakdown of the procedure. I will also explain how we integrate this advanced intervention at our clinic, Injury Medical Clinic PA. Our unique multidisciplinary model combines the medical oversight of our Medical Director, Dr. Maria Guadalupe Cardenas, MD, with my expertise in chiropractic and functional medicine. Understanding Adhesive Capsulitis (Frozen Shoulder)

Before we discuss treatment, it’s important to understand what happens inside the shoulder joint during adhesive capsulitis. The shoulder is a ball-and-socket joint (the glenohumeral joint), enclosed by a flexible connective tissue structure called the joint capsule. In a healthy shoulder, this capsule is loose and pliable, allowing for an extensive range of motion.

In frozen shoulder, this capsule becomes inflamed, thickened, and fibrotic. Adhesions, which are bands of scar tissue, form within the capsule, causing it to shrink and tighten around the humeral head. This process severely restricts movement and causes significant pain. The condition typically progresses through three stages:

  1. Freezing Stage: Characterized by the gradual onset of diffuse, severe pain, especially at night. Range of motion begins to decrease. This inflammatory phase can last from a few weeks to several months.
  2. Frozen Stage: Pain may start to lessen, but stiffness becomes the predominant symptom. The shoulder’s range of motion is dramatically limited, making everyday activities like dressing or reaching for an object extremely difficult. This stage can last for several months to a year.
  3. Thawing Stage: The shoulder’s range of motion slowly begins to improve. This stage can be very slow, sometimes taking years to resolve completely without intervention.

The goal of treatment is to interrupt this pathological process, reduce inflammation, break down adhesions, and restore normal joint mechanics.


Hydrodistension: A Targeted Intervention for Frozen Shoulder

Hydrodistension, or hydroplasty, is an elegant and highly effective procedure designed specifically to combat the capsular contracture of frozen shoulder. The fundamental principle is to use hydraulic pressure to physically stretch and ultimately rupture the shrunken, fibrotic joint capsule. By injecting a large volume of sterile fluid directly into the glenohumeral joint space under ultrasound guidance, we can mechanically break the adhesions that are restricting movement.

To illustrate this state-of-the-art technique, I’d like to share the work of a primary care sports medicine physician. Their meticulous, evidence-based approach provides a clear window into how this procedure is performed.

The image below shows a posterior view of the shoulder joint via ultrasound. We can clearly identify the deltoid muscle, the infraspinatus tendon, and the posterior joint capsule. Critically, we see the needle precisely positioned within the joint space—between the capsule and the humeral head—ready for the injection. This real-time imaging is what makes the procedure both safe and exceptionally effective.

The Three-Step Procedural Protocol

The protocol is methodical and designed for maximum patient comfort and procedural success. It involves three key injections.

1: Suprascapular Nerve Block

  • The “Why”: The first step is not to treat the capsule but to manage pain. The suprascapular nerve provides sensory innervation to about 70% of the shoulder joint. By performing a nerve block at the spinoglenoid notch—a small anatomical passageway where the nerve is easily accessible—we can induce profound, temporary anesthesia. This makes the subsequent, more intensive part of the procedure far more tolerable and provides significant post-procedural pain relief.
  • The “How”: Using ultrasound guidance, a needle is directed to the spinoglenoid notch, located just medial to the glenohumeral joint. A local anesthetic, such as a combination of lidocaine (fast-acting) and ropivacaine (long-lasting), is injected around the nerve.

2: Numbing the Joint Capsule

  • The “Why”: Before injecting a large volume of fluid, the posterior entry point and the joint itself are numbed. This ensures that the patient does not feel the placement of the larger-gauge needle used for the main injection.
  • The “How”: A small-gauge needle is used to inject a buffered lidocaine solution directly into the subcutaneous tissue and then into the posterior glenohumeral joint space. Adding sodium bicarbonate to the lidocaine helps neutralize its pH, reducing the stinging sensation during injection.

3: The Hydrodistension Injection

  • The “Why”: This is the procedure’s therapeutic core. The goal is to rapidly inflate the joint capsule with enough fluid to overcome its tensile strength, causing the adhesions to tear and the capsule to stretch.
  • The “How”: A larger, 18-gauge needle is guided into the exact same intra-articular position under ultrasound. This needle is connected via IV tubing to a large syringe containing the injectate. A typical mixture includes:
    • Local Anesthetics (e.g., 10 mL Lidocaine, 10 mL Ropivacaine): To provide ongoing pain control within the joint.
    • Sterile Saline (e.g., 30 mL): This makes up most of the volume needed to create hydraulic pressure for distension.
    • Corticosteroid (e.g., 1 mL of Kenalog 40 mg/mL): This powerful anti-inflammatory agent helps calm the intense inflammation driving the fibrotic process.

As we inject the fluid, we can watch on the ultrasound screen as the posterior capsule lifts away from the humeral head. The capsule visibly expands like a balloon until it reaches its breaking point. At that moment, the screen often shows a sudden deflation as the fluid ruptures the capsule and extravasates into the surrounding tissues. This “pop” signifies a successful procedure—the mechanical restriction has been broken.


The Role of Integrative Chiropractic Care Post-Hydrodistension

Hydrodistension is a powerful catalyst for recovery, but it is not the end of the journey. Breaking the adhesions is only the first step. Without proper follow-up care, the capsule can re-adhere, and the underlying biomechanical faults that may have contributed to the problem will persist. This is where integrative chiropractic care becomes essential.

My clinical observations at our El Paso practice have consistently shown that patients who engage in a structured post-procedural rehabilitation program achieve faster, more complete, and more durable results.

Our approach focuses on several key areas:

  • Mobilization and Manipulation: Immediately following the procedure, it is crucial to capitalize on the newly created range of motion. We use gentle, specific chiropractic mobilization techniques to guide the glenohumeral joint through its full, pain-free range. This helps prevent scar tissue from reforming and helps “retrain” the joint to move correctly. We also assess and address any associated restrictions in the cervical and thoracic spine, as dysfunction in these areas can alter shoulder mechanics and contribute to impingement.
  • Soft Tissue Therapies: We employ techniques like Active Release Technique (ART) and Graston Technique to address residual myofascial restrictions in the muscles surrounding the shoulder, such as the infraspinatus, subscapularis, and pectoral muscles. Tightness in these muscles can create abnormal pulling forces on the joint, and releasing them is vital for restoring balanced movement.
  • Neuromuscular Re-education and Rehabilitation: After a period of immobility, the neural pathways that control shoulder movement become dysfunctional. We use specific exercises to re-establish proper motor control, strengthen the rotator cuff and scapular stabilizer muscles, and restore the correct scapulohumeral rhythm—the coordinated movement between the shoulder blade and the upper arm bone.
  • Functional Medicine and Anti-Inflammatory Support: Frozen shoulder is fundamentally an inflammatory condition. As a functional medicine practitioner, I investigate potential systemic drivers of inflammation, such as insulin resistance, nutrient deficiencies (e.g., Vitamin D, magnesium), or gut dysbiosis. By addressing these root causes through targeted nutritional protocols and lifestyle modifications, we can help modulate the body’s inflammatory response and reduce recurrence risk.

Conclusion

By combining the powerful, targeted intervention of hydrodistension with a comprehensive, integrative rehabilitation plan, we guide the patient from pain and restriction to full, fluid, pain-free function. This is the promise of truly collaborative healthcare.
